Dotphrases for assessment and plan are often used in clinic and urgent care settings. They are designed to be used in the SOAP note template and conveys to the reader what your assessment and management plan is for the patient.
A&P is different than MDM which is typically used in an emergency room setting, although in some acute-care visits MDM may be an appropriate dotphrase to use instead. A&P dotphrases are for issues that have a clear diagnosis. For example, you would chose A&P for a routine hypertension visit in the clinic. However, you may chose MDM Hypertension if the eitology of hypertension is less clear and you have a larger workup planned.
